Wisconsin Medicaid offers two programs to help seniors retain their independence and live in the community rather than in a nursing home. 

The Family Care program provides seniors and disabled residents with long-term care services. Once enrolled, participants partner with a managed care organization to assess their individual needs and develop a customized health care service plan. Services are provided where the participant lives, which can be an assisted living facility or a personal residence.

To be eligible for the Family Care program, seniors must be at least 65 years old or disabled, require long-term care services and meet the Medicaid income limits.

Once enrolled, seniors receive benefits such as:

  • Primary health care coordination
  • Assistance with activities of daily living such as bathing, eating and dressing
  • Adult day care
  • Mental health services
  • Speech, occupational and physical therapies
  • Skilled nursing
  • Transportation to medical appointments
  • Daily living skills training


The Include, Respect, I Self-Direct program provides Medicaid-eligible seniors and disabled adults with a budget to direct their own care based on individual need.

To be eligible for IRIS, applicants must meet the following criteria:

  • Over age 18, a frail senior or disabled adult who requires nursing home-level care
  • Eligible for Medicaid
  • Live in a residential care apartment, adult family home or private residence in Wisconsin


Program members can allocate their budget to pay for goods and services that meet their needs and long-term goals.

Wisconsin Supplemental Security Income Exceptional Expense Supplement

  • Have a monthly income of less than $772 for individuals and $1,158 for couples
  • Have total assets of less than $2,000 per individuals or $3,000 per married couple, excluding a primary home, burial plots and one personal vehicle


SSI recipients may qualify for SSI-E benefits if they require at least 40 hours of long-term support services per month and pay for their own household expenses in a private residence. They may also qualify if their expenses exceed the SSI-E payment level and they reside in one of the following facility types:

  • 20-bed or smaller community-based residential facility
  • Certified residential care apartment complex
  • Certified adult family or foster home
