Genworth’s 2018 Cost of Care study examined senior care costs throughout the United States. The study shows that at an average price of $5,448 per month, assisted living in Naples costs significantly more than the state ($3,500) and national ($4,000) averages. In fact, prices here are the highest in Florida with prices in nearby areas ranging from $3,350 in Miami to $4,000 in Punta Gorda. 

Statewide, assisted living costs start at $2,808 per month in the Lakeland area. The northernmost region of the state is the most expensive, with prices ranging from $4,096 in Pensacola up to $4,425 in Jacksonville. 

The cost of senior care is largely based on the level of care provided with each service, and that’s especially true in Naples where semiprivate nursing home care costs an average of $10,319 per month. This price includes room and board along with around the clock medical care in a state-inspected nursing facility.  Assisted living, which also includes room, board and most overhead expenses, costs $5,448 per month, while 44 hours of weekly in-home care ranges from $4,195 to $4,576. 

 

Medicaid was established on July 30, 1965 to ensure needy people had access to basic health services. It’s jointly funded by federal and state governments and administered at the state level. By law, every Medicaid plan must include essential benefits, such as hospital and nursing facility care, primary physician services, X-rays and lab tests. 

Approximately 88% of seniors and people with disabilities in Florida qualify for no-cost Medicaid because they have low income and few countable assets. Seniors who collect Supplemental Security Income benefits automatically qualify for SSI-Related Medicaid coverage. 

Qualifying for Medicaid Long-Term Services

To qualify for long-term Medicaid services, seniors must have a monthly income of $2,313 or less when applying as an individual or $4,626 when applying with a spouse. If only one spouse in a married couple needs Medicaid, the non-applicant’s income is disregarded.

Single applicants can own up to $2,000 in countable assets, and couples applying together can own up to $3,000 in joint countable assets. When only one spouse is applying for Medicaid, the non-applicant is entitled to 50% of all joint assets up to a maximum value of $126,420. 

Some assets are excluded under Medicaid rules, such as one vehicle, clothing and furniture and a prepaid burial contract. A house, condominium or mobile home worth $585,000 or less that’s owned and occupied by the applicant and/or their spouse is also exempt. 

Qualified Income Trust

Seniors who meet the Medicaid asset limits but are ineligible due to excess income may be able to enroll in Medicaid by setting up a Qualified Income Trust. 

Commonly called a Miller Trust, a QIT is an irrevocable legal agreement between the applicant and the state. The applicant agrees to place their excess income into the trust, and the state is named as the beneficiary. Money from the trust can only be used towards specific preapproved expenses and a modest monthly personal needs allowance.

Upon the passing of the QIT holder, the state recoups all Medicaid costs paid on behalf of the applicant by collecting any money remaining in the trust.

Statewide Medicaid Managed Care Long-Term Care Program

The Statewide Medicaid Managed Care Long-Term Care program provides home and community-based services that allow seniors to age in place in their community or assisted living facility rather than in a nursing home. 

SMMC LTC funds services and supports that are comparable to those offered in a nursing facility, such as: 

• Attendant care
• Personal care
• Medical supplies
• Mobility equipment
• Transportation
• Intermittent nursing
• Occupational, physical and speech therapy
• Adult day health programming
• A personal medical alert system 
• Respite services
• Case management

In accordance with federal law, all Medicaid-enrolled seniors who require long-term care services must complete preadmission screening known as the Comprehensive Assessment and Review for Long-Term Care Services. This assessment evaluates the applicant’s current and projected care needs, and recommends the most appropriate, least-intrusive placement based on those needs. 

If the CARES assessment results in a referral to the SMMC LTC, services are assigned based on the individual care needs identified through the assessment.

VA Aid and Attendance and Housebound Enhanced Monthly Pension

The VA Aid and Attendance and Housebound programs are two special monthly pensions that provide eligible veterans and survivors with cash benefits that can be used to pay assisted living costs. 

In order to qualify for either benefit, applicants must be currently eligible for the regular VA pension. Applicants who were previously denied the regular VA pension due to excess income may now be eligible because these special pension programs are based on a higher income limit. 

To apply for Aid and Attendance, applicants must: 

• Live in a nursing facility as a result of physical and/or mental deterioration

• Need help from a caregiver to perform activities of daily living

• Be restricted to bed due to a chronic illness or disability

• Have concentric contraction of the visual field that limits the field of vision to 5 or fewer degrees

• Have 5/200 or worse eyesight when wearing prescription lenses

To apply for Housebound, applicants must:

• Have a single disability rated at 100% that restricts the applicant to their residence most of the time

Eligible applicants may receive either the Aid and Attendance or the Housebound benefit.

Optional State Supplementation

The Florida Optional State Supplementation program helps eligible seniors pay the room-and-board portion of their assisted living costs by topping up the monthly Supplemental Security Income payment. 

To qualify for OSS, SSI recipients aged 65 and older must reside in an assisted living facility due to disability or loss of function. Eligible applicants may receive up to $78.40 per month, while couples may receive a maximum of $156.80 in monthly OSS payments. The OSS is added to the monthly SSI benefit.
